Wound Care Center

SCOPE

Wound and Stoma Therapy is an outpatient program for the treatment of chronic non-healing wounds. The department is located in the outpatient center at Memorial Hospital Jacksonville. Wound Care is provided by a multidisciplinary team including general and specialty surgeons, and podiatrist. The nursing staff consist of three registered nurses specially trained in case management and are nationally certified in wound care as Certified Wound Care Nurses (CWCN). Enterel Stoma Therapy (ET) nurses are available for consult as needed for stoma care.

SERVICE AND GOALS

Our staff is dedicated to the healing of chronic wounds: Wounds that have not shown any improvement in four weeks, or wounds that have not healed in eight weeks. Goals include decreasing the wound healing time, prevention of future and/or recurrent wounds.

Services start with a complete evaluation, with the underlying etiology to be identified if able. Our convenient hospital location allows us to treat every type of wound. Plan of care is established and includes, if applicable:

. Laboratory tests
. Vascular studies
. Infection control
. Patient/family education
. Surgical interventions
. Orthotics
. Protective/Pressure devices
. Case Management
. Communication with referring physicians


PERFORMANCE IMPROVEMENT


Performance improvement is an integral part of the Wound and Stoma Therapy Department. The primary purpose is to heal our patients' chronic wounds, prevent future wounds by education, and increase their quality of life. Patients' wounds are evaluated every visit for progress, or lack of, and are addressed at that time for a change in plan of care by the physician and the case manager. Wounds that have not healed by 50% in eight weeks and healed by 16 weeks are presented to the medical director for peer review.

Customer Satisfaction is important to us!


Satisfaction surveys are given, or sent in the mail, to all patients at eight weeks of therapy, 16 weeks, every eight weeks thereafter and on discharge. Surveys are also available in the waiting room.
